The first 90 days after a stroke are the most critical window for recovery. The brain is rebuilding connections, and the care environment during this period directly affects how much function returns — and how quickly.

What recovery actually looks like

Stroke recovery is not linear. progress happens, then plateaus, then sometimes resumes in unexpected ways. The brain’s plasticity — its ability to rewire around damaged areas — is greatest in the weeks and months immediately following a stroke, which is why this window is so important.

Most stroke survivors are discharged with some combination of home health services and an expectation that family or in-home caregivers will fill the gaps. Those gaps are significant.

What home health covers — and what it doesn’t

Home health services cover skilled clinical needs: a nurse, physical therapist, or speech therapist visiting a few times per week. What they don’t cover: daily personal care — bathing, dressing, meal preparation, medication reminders, transportation to follow-up appointments.

The highest-risk issues in early stroke recovery

Falls: Weakness, balance problems, and impulsivity dramatically increase fall risk post-stroke. The home environment needs assessment and modification.

Medication management: post-stroke regimens are complex and critically important. Missing doses can precipitate a second stroke.

Depression: Affects up to a third of stroke survivors and significantly impairs recovery motivation.

Communication barriers: Aphasia affects roughly a third of stroke survivors. A patient, trained caregiver makes an enormous difference.

How Care partners supports stroke recovery

Our caregivers receive specific training for post-stroke care, including mobility assistance, adaptive communication strategies, and recognizing warning signs that warrant urgent medical attention. We can often have a caregiver in place within 24 hours of discharge.
