Enhanced Care Management — ECM — is California’s answer to that problem. It’s one of the most valuable and underutilized benefits available to eligible Medi-Cal members.

What is Enhanced Care Management?

ECM is an intensive care coordination service offered through California’s Medi-Cal managed care plans as part of the CalAIM initiative. It assigns a dedicated Lead Care Manager to a Medi-Cal member — a single point of contact who coordinates across all of that person’s providers, services, and support systems.

What the Lead Care Manager actually does

A Lead Care Manager meets with the member in person, develops an individualized care plan addressing medical, behavioral health, and social needs, coordinates with the member’s physicians and ensures follow-through on referrals, connects the member and family to community resources, monitors for changes in condition, and advocates for the member within the healthcare system.

Who qualifies for ECM?

ECM targets Medi-Cal members with the highest complexity and greatest risk of poor outcomes, including adults with multiple chronic conditions and frequent hospitalizations, individuals experiencing homelessness, adults transitioning from incarceration, individuals with serious mental illness and co-occurring conditions, and adults with high emergency department utilization.

In Orange County, eligibility is determined through CalOptima.

ECM and in-home care

ECM and in-home personal care are complementary, not competing. ECM handles coordination — making sure the healthcare system is working for the member. In-home care handles daily support — making sure the member is safe, eating, and managing their daily life.
